New Patient Registration

Anyone wishing to register as a patient with the practice should speak to one of the reception staff to arrange for a completion of the appropriate form.  All new patients will be asked to come to surgery for a new patient check.  This will enable us to put onto the system any medication etc.

We would ask that you try to attend the surgery within your area.  However, there may be times when it is more convenient for you to attend the other surgery,  please telephone beforehand to check on times and availability.

Practice Area

The practice accepts patients residing in the following areas: Thorpe Hesley, Brampton, Bierlow, Harley, Scholes, Wentworth, Nether Haugh, Swinton, Dalton, Rawmarsh, Kimberworth and Mexborough.
