PreRegistration

If you would like to register with our practice then we ask all new patients to collect a paper registration form from either surgery. We aim to have you registered on the system within one week. Please note the process of receiving your medical records from your previous GP can take several weeks.

Once registered we ask all new patients to book in for a new patient medical check with one of our healthcare assistants. During the appointment you will be briefly asked about your family history, current medications and a few minor assessments will be carried out including your height, weight and blood pressure.

We ask all new patients to bring some form of documentation to your new patient medical appointment that clearly states the medications you are currently taking. The healthcare assistant will add the medication onto your record during your appointment. You may want to nominate a different pharmacy upon registering, if so please discuss this with a member of reception.
